Dexcom has teamed up with Promatica Digital to set up their PRISM tool in your local ICS.
The PRISM tool uses customisable patient markers to provide prescribers in your ICS with the clarity to readily identify and authorise priority diabetics who require intervention with rtCGM, and see real-time progress updates in dashboard format.
Promatica can then provide expert clinicians to implement the service.
This service is fully funded by Dexcom.
What insights does the PRISM tool identify?
The Promatica Digital PRISM tool is designed to review your patient databases and find patients who meet certain criteria and parameters. These parameters can be customised according to your local needs and targets.
Once patient cohorts have been identified, the PRISM tool enables clinicians to review patients and invite them for further intervention, such as information about rtCGM and/or follow-up discussions during upcoming diabetes/HbA1c review appointments.
